Your First Visit

Remember to bring your insurance card with you. Please bring a referral form from your primary care physician if your managed care plan requires it. You may be financially liable for all medical services if appropriate referral or insurance information is not attained and provided prior to your appointment.

Please check with your insurance company regarding benefits to avoid financial problems. We have found that participation with the structure of HMO's and PPO's change from time to time. Please check with us periodically to see if we currently participate with your plan.

You are responsible for the annual deductible on your insurance plan. We will be happy to submit all charges to your insurance company. All co-pays are expected at the time of the office visit.

Remember

• Please cancel appointments 24 hours in advance so that we can accommodate other patients.
• Bring all current medications, x-ray reports, and relevant information to your appointment.
• Inform us before your appointment of any insurance or address changes.
